New Patient Registration

To register as a patient, please bring your medical card to the surgery. If you do not have a medical card, we will give you a form to fill in. Alternatively you can download them below:

Patient questionnaire

New patients (over the age of 16) will be offered a health check. If you are unable to come to the surgery because of ill health, but wish to accept the offer of a health check, this can be arranged as a home visit.

You may consult any of the doctors. You may also express a preference for a particular doctor and we will try to accommodate you, wherever possible. We suggest that if possible you try to see the same doctor for follow–up appointments in any particular episode of illness – but please do not wait if your problem is painful or worrying. If your own doctor is busy, see one of the other partners who can then pass you back to your own doctor later.

Change of Address/Telephone Number

Please inform us immediately if you change your address or telephone number (including ex-directory numbers). You will also need to let the hospital know of any changes if you are attending outpatients or awaiting in-patient treatment.
