We are delighted to announce that Academy Medical Centre will continue to implement change and to deliver new models of care that will benefit patients.
At Academy Medical Centre, the planning for our new model is progressing in the same way as before. We will deliver care through small multi-disciplinary teams of health professionals, each team comprising one GP, practice nurses, healthcare assistants and administrative staff. Patients will be allocated to one of 5 small teams, each aiming to deliver tailored care to meet the needs of the individual. Coordinating care in this way will offer patients continuity with a group of care providers that they will get to know well. We will be extending the range of services available in the practice to include health psychology and social prescribing, with the aim of delivering a primary care service that comprehensively manages your medical condition and also addresses some of the wider factors that can affect health & wellbeing. All care will be coordinated by your team.
Currently Academy Medical Centre is undergoing a transition phase. The premises have been upgraded to accommodate our new small team way of working and to deliver our ambitions for our new model of care. All patient requests will be assessed by a case management team who will ensure that your needs are responded to promptly by the most appropriate health professional. This means that, when you call, your request will be case managed by a doctor or nurse to ensure that your needs are met.
During this transition phase we greatly appreciate your patience and cooperation as we move towards our new models of care.
Once premises plans have been put in place, we will fully launch our new model of care in Summer 2016. We will write to you to confirm which small team you have been allocated to, how to access the new service, what to expect when you call, and how to give us your feedback. At the same time, we will also provide information and updates on our website (www.academymedicalcentre.co.uk) and in the surgery waiting area.
